Your First Appointment

Before you come to your first appointment…

*Prior to meeting, initialed and signed Informed Consent and Financial Agreement must be on file. These can be found here. Please email these to your therapist, once initialed and signed.

Additionally, if you are seeking therapy on your own, please print, complete and bring to your appointment a Psychotherapy Intake Form. If you are coming as part of a relationship, please print and complete a Couples Intake for each member of the relationship.

If you would like your therapist to interact with anyone else about you (doctor, dietitian, family member, etc) a Release of Information must also be provided to your therapist before this can take place.

If you have a psychiatrist, primary care physician, or referring therapist, you may also bring a Care Coordination Form, after it has been completed by this professional.

You will receive an email from Affinipay requesting securely stored credit card information. Intake session fee of 185 will be charged the day of our session.
